Replacing an exhausted item

If my weapon is exhausted and I draw a weapon treasure to replace it, does the new weapon enter play refreshed or exhausted?

New Treasure cards enter play refreshed even when replacing an exhausted card of the same type.

There's a sidebar on page 23 that explicitly calls this out- not sure how I missed it.
